The clinical services program provides 24/7 support for patients, providing peace of mind for patients & families. Examples include: Chronic Care Management & Remote Patient Monitoring
Job Requirements:
- Understanding the product completely and guiding the patient, Caregivers about the product
- Troubleshooting the product and guiding the patient on questions
- Logistic Management and Chat support.
- Remote Patient Monitoring devices & alerting.
- Plan of care coordination with PCP
- Proactively identify home-based interventions
- 24/7 Triage and Clinical Decision support by phone or Video.
- Perform direct care management and remote patient monitoring service as needed.
- Provide medical direction and administration including, but not limited to, developing and overseeing the implementation of clinical practice guidelines and delivery of clinical services.
- The technology platform enables virtual connection to get eyes on members for telemedicine visits, group activity, and other video-based uses.
- Digital Health Check-ins from patients & in-home caregivers.
- Targeted messaging & educational content
- Integrated video visits.
- Create strong relationships with clinical and non-clinical team members
Professional Attributes:
- Good Written and communication skills. Should have the experience interacting with US customers
- Current knowledge of non-complex and complex chronic care management, and population health best practices.
- The remote patient monitoring program experience is also desired.
Technical Experience:
- Pre-visit planning workflow to ensure care completion prior to visit whenever possible
- After visit summary review with patients whenever appropriate
- Involving the patients in activities to improve their health (patient engagement)
- Educating the patient about self-management tasks they can undertake to gain greater control of their health status
- Works with IT resources to facilitate registry reporting at the site & documentation
- Works with physician leadership to define quality measure/outcome reporting process
- Works with physician leadership to develop protocols and point of care reminders using nationally recognized measure/outcomes
- Serves as a resource to clinical staff and providers to establish quality goals using reports
- Maintains strict confidentiality and follow HIPPA regulations
- Treats staff, physicians, NPs/PAs, visitors, patients, and families with dignity and respect
- Participates in professional development activities
- Performs other related work as required.
